Things tend to escalate, but in a hypothetical in which they don't it might be ok. You also have to take into account the shame and social stigma that go along with self harm. Lastly there are just more effective and healthy ways to manage emotions.

Because people who self-harm frequently are not doing it safely, it has lasting damage, and it’s typically indicative of an individual who lacks outside resources to help them. Particularly for teens, ignored self-harm can escalate into even riskier behaviors.

There’s a massive difference between reckless self harm by depressed teenagers and deliberate but safe “harm” done as part of a kink by a mentally well adult.

More social programming. We are taught to not harm another or ourselves. What you describe is not really harm though? Much like the person who is turned on sexually by strangulation. Harm is the wrong word to describe it. I'm also at a loss for a word that fits.

My answer is that there are far more safer, less potentially costly (medical bills), forms of stress management - meditation, therapy, exercise, etc.
