Like... something that actually doesn't respond to, or is made worse by, the strategies that the untrained person likes to use and has found works on most people. I'm thinking of one example of someone with borderline personality disorder.

Yea, and it takes a lot to really get to that point. This would be another indicator of a high skill therapist - being able to be resolutely non-judgmental in a high outlier situation

Therapy was actually going to be my first idea when you 1st posed this question lol. Being prepped to screen and accommodate for low occurrence disorders, as well as potentially other high impact events (so essentially ability to conceptualize a case well) is 1 indicator of skill

The ability to be resolutely non-judgemental (as mentioned by someone else) and able to navigate highly emotional situations are others. Ability to foster a strong therapeutic alliance (which involves identified goals) is another one that comes to mind.
