Voluntarily when I was 13. Was deeply religious til 16 then started losing various levels of the brainwash over the next several years. Would still go to church for the social aspect for years after I stopped believing.

It was as voluntary as many other things that you'd rather not do, but still do because you're 11yo and isn't used to refuse your parents, so... semi-voluntary for me. I mean, I could say no and they'd probably be OK with that. But it wasn't really an option for me then.

Yes, with the consent of my family who are charged with my upbringing, and with no retoractive issues to be had once I learned "What was done to me"...... *barf* Let's maybe not completely destroy the obligations of family to do what is best.
