Am retired emergency management specialist with BuRec. Assessment that "dams don't fail with warning" is incorrect. Even catastrophic failures are preceded by 'warning' signs prior to failure. Failure to monitor 'warning' signs & take appropriate action is the issue.

It does! As soon as people die, the survivors can litigate. Years and years later, they get a settlement while the perps admit no wrongdoing. So they can do no wrong again. Nice trick.
