It's more complicated for big cities to get their votes in because they are dealing with hundreds of thousands of more votes. These big cities are generally heavily Democratic compared to rural areas with few votes. This isn't the first time this has happened, wont be the last

Rank choice is different than regular voting. Someone who is ahead and the end of the first count could, and apparently did, lose. There is a great This American Life podcast about rank choice.
