After all, "facial feminization surgery" is covered as "gender affirming." If I want a facelift because my brain is no longer congruent with my aging female body, and I need it to feel "authentic," how can they deny that? Quite serious here.

Standard idea of "woman" is full, pert breasts. After menopause, many women lose that, feel...dysphoric; incongruent with the idea they have of themselves (their "gender"). How could an insurance company deny coverage for that, but not for a 50-y-old man getting implants?
