yes--the separation btw urban and commuter rail is what forces the need for a single NS tunnel for regional connectivity, rather than using through-running on the subways to get four such tunnels. Ideally, commuter rail rolling stock and frequency would be similar to subway

At a bare minimum: differences in seat arrangement, standing room, and door count do not require any differences in carriage dimensions, loading gauge, power source, etc--ought to be interoperable with subways even if slightly different inside

Unless you have platform edge doors, when different door layouts do cause problems. Or a legacy system with a very tight loading gauge, small carriages, live rails, rubber tyres etc (as with both Paris and London).

MBTA *is* a legacy system which is why interoperation is impractical without some sort of drastic systemwide overhaul--not something likely to happen in the near future. When I'm talking about interoperation and subway/regional differences, I'm talking about ideal, not legacy

Fair. Though platform edge doors, as we see on new subway platforms, mean you can't have very different door layouts for different trains on the same line.

not necessarily--just means that you'd need to indicate which doors will open for which trains, easy enough with color coding (this is common in Japan, not for platform doors, but for door position indicators which are painted onto platform edges)

hypothetical example = there's one color door indicator which marks where doors on a 10-car train w 4 doors per car will be, another color for 10-car 6 door trains, another color for 6-car 3 door trains, etc etc

harder with doors than with simple stripes painted onto the platform, yes--but could work, it all depends on the actual measurements and differences in question. certainly easier with things like common car length!
