My impression is that Boston did a much better job than most when it came to its Charlie Card, which (iirc) was also one of the first such cards to be launched domestically--but even they haven't figured out how to get it to work on the commuter rail...

The DC cards haven't always been this bad! I moved to DC in 2002 and there was a noticeable slowdown once they reprogrammed the machines to include the silver line fares.
