  3. @DebHazeldine I think it is a problem when systems become defensive. Not always easy to overcome this.
  4. @DebHazeldine The C diff case was final straw for lady who had been dying for 5 months. I am not sure if it was listed.
  5. @DebHazeldine it was at the time when the awareness of issue was growing nationally. MRSA was listed as a contributory cause of death.

  7. @katemasters67 the word that worries me here is "force". I think that to make it work we need softer words.
  8. @katemasters67 The key to making a complaints system work is communication skills - & there are not enough people who have them.
  9. @katemasters67 dont want to be depressing - but this was all done in 2009 too! Drawing up new systems is the easy part.

  11. @katemasters67 rather wish I had had time to get involved with this - have a background in complaints handling. - but no time to do it all.

  14. @katemasters67 yes. This was one of the things that interested me at PI. Different bits for different purposes - none of which exactly fit
  15. The data on things like pressure sores is becoming much more helpful - Most of the cases came in with them from nursing homes.
  16. The level of detail that is available to the board at Mid Staffs is impressive. e.g. there were 3 C diff cases - but know none related.
