@ Ctrl-Alt is used by Parallels to escape Windows app mouse+kbd. Go into Parallels key mappngs setup and change it to Cmd+Ctrl+Space.

@ Tell Hugh Tramadol is quite good, lived on it for 1yr but now immune to it. They all have the same problem, try lactulose.
